What Does the Research Say About Ultherapy Prime?

HALO already covers Ultherapy Prime from a few angles — our FAQ post answers candidacy questions, and we’ve compared it directly to XERF and OligioX. What’s missing is the evidence itself: what the actual clinical research on microfocused ultrasound shows. This piece fills that gap.

How Ultherapy Prime Works

Ultherapy Prime uses microfocused ultrasound with visualization (MFU-V) to deliver targeted thermal energy beneath the skin’s surface — including the SMAS layer, the same structural layer surgeons tighten during a facelift. Real-time ultrasound imaging lets the provider see the tissue being treated before delivering energy, rather than treating blind. That thermal energy triggers a wound-healing response that produces new collagen over the following months, gradually lifting and tightening skin without cutting it.

What Does the Research Show?

The foundational study on this technology, published in the Journal of the American Academy of Dermatology in 2010, followed 35 evaluable subjects who received microfocused ultrasound treatment to the face and neck. Three masked clinician raters judged 86% of subjects (30 of 35) to show a clinically significant brow lift 90 days after a single treatment (P=.00001), with an average brow height elevation of 1.7mm. Side effects were limited to transient mild redness and swelling in most responders.

A 2019 study in Clinical, Cosmetic and Investigational Dermatology looked more closely at what happens to the skin itself: researchers measured skin elasticity in 22 patients at baseline, day 3, and weeks 4, 12, and 24 after a single treatment. Elasticity initially dipped at week 4 — consistent with the tissue remodeling process — then significantly increased by weeks 12 and 24. No adverse events occurred over the full 24-week follow-up, and skin barrier function (hydration, water loss, redness) remained stable throughout.

How Does This Compare to HALO’s RF Devices?

The biggest difference is the energy type and depth: Density, XERF, Xtherma, and OligioX all use radiofrequency energy to heat the dermis and subcutaneous tissue. Ultherapy Prime instead uses focused ultrasound, which can reach the SMAS layer — deeper than most RF devices target, and the same layer addressed surgically in a facelift. That’s part of why Ultherapy Prime is often positioned as the closest non-surgical option to a facelift, while the RF devices are typically used for surface-level tightening and texture.
